Firm Data Change Requests
In order to keep our records accurate, we must have proper information for your firm. To that end, we would like to request that you notify the Needles office staff with any changes in your firm's contact information, including: firm name, physical address, mailing address, fax number and phone number. If you have recently made changes to any of this information, please complete the appropriate fields and submit your request.
Note: After properly completing and submitting this on-line form a Needles technician will contact you to update the information contained in your firm's Needles System.
